Instance list

For beta 7, I wanted to make a list of the instances, their approximate level ranges and their coordinates as well as have a screenshot of their location. I wanted to have this for our reference so we won't have problems finding them again, or for people in our guild that want to go and do them but don't know where they are. Anyone interested on helping me?

I found Fall of Lantern Hook in...Lantern Hook. There's a fountain in the cave and that's where the portal is. Mobs were 44+ elites. There wasn't a portal icon on the map so I couldn't take a screen shot for you.

I think Runic Descent is lower, like 38-40

Do the "+" and "++" have any specific meaning? Or just:

+ = well geared from previous instances and quests.

++ = well geared from expert level instances.

Or is it more of a vague "+" is only for the badasses, and ++ is for the most PRO OF PROS!?
